Use this secure form to register into the ID Card Website System as an executive of the organisation.

Zonal executive tenure expires after 4 years and national executive tenure expires after 4 years based on the admin settings.
First Name is required.
Surname is required.
Scope of Operation
Select one option only.
Choose a zonal area.
Position is required.
Upload a clear JPG or PNG passport photograph.
Passport Photograph is required.

Form Guidance

  • Complete First Name and Surname. Middle Name is optional.
  • Select exactly one scope of operation
  • Choose a zonal area only when Zonal is selected
  • Enter the executive Position before submitting
  • Zonal and national executive expiry dates follow the tenure values configured in admin settings
  • Upload a clear passport photograph in JPG or PNG format
